SCHOOL READINESS—an evaluation for 5 and/or 6 year olds.

I’m doing school readiness evaluations—please read the following:  

FIRST SESSION

 

In this first session I saw the parent/s/guardian of the child. Questions about the child are asked and the parent/s/guardian can use this session also to inform me about some confidential aspects about the child, in order to understand or help the child. It is not necessary to bring the child along. A questionnaire is to be completed by the parent/s/guardian at home whereby some background information is asked. Please remember to deliver this questionnaire when you bring the child for the evaluation.  

 

SECOND SESSION

 

In the second session I saw the child for a series of tests and/or diagnostic games. (If the child is anxious the parent/s/guardian can stay outside and wait until all is completed).

· Complete Group Test for 5 or 6 year olds (visual discrimination; place in space; fine motoric coordination; verbal; classification; numbers; Gestalt)

· Several diagnostic tests; DAP; KFD; etc.

 

Please bring the completed questionnaire to the second session because I need this to integrate it with the findings of the second session.

 

THIRD SESSION

 

After about a week or two—which gives me time to mark and evaluate all the findings—the parent/s/guardian comes back to discuss the report.

It isn’t necessary to bring the child along.

 

TARIFF: R 2100,00 for the 3 sessions. Payable with booking.

 

More sessions: If needed, more sessions could be done, for example to address trauma, family circumstances, etc. at R550 per 50 minute session.
